It has become increasingly noticeable that a lot of prescriber are including patients' conditions for which the medication has been prescribed for.It started initially with a few medicines but now almost all prescriptions contain the medical conditions and most are even abbreviated,example " for HT. MI, PMI or full wording like for prostate,breast cancer ".Labels already carry a lot of information and in my opinion they are being turned into a mini 'PMR'.Anyone having a glance at a label can with no doubt know the full medical history of a patient.Do prescribers expect pharmacists to print all this information on the label even abbreviations which the patient clearly do not understand their meaning.Are we exposing patients to potential breach of confidentiality by being too overzealous in our practice?
