Hertfordshire PCTs are undergoing a major service redesign programme with significant opportunities for pharmacy to contribute. We are expanding and revising intermediate care services and are looking to provide world class pharmacy and medicines management services to these patients. Two new posts have been created and we are looking for innovative pharmacists to develop and provide a clinical pharmacy service and to build effective relationships with primary and secondary care practitioners and other agencies to ensure patients medicines management needs are met.
In addition you will be part of the specialist team of pharmacists who support GP practices to deliver cost-effective prescribing, and to liaise with community pharmacists to support Practice Based Commissioning.

All posts will have opportunities to gain experience and training in all activities of the team to have a portfolio of responsibilities leading on specific areas of therapeutics or service for the pharmacy team. If you have sound clinical knowledge, imagination and experience and a proven track record of managing change, motivating others and meeting deadlines we are keen to hear from you. Primary care experience is not essential as training will be provided. Car driver essential, or access to a car (unless you have a disability as defined by the Disability Discrimination Act 1995).
All posts: 37.5 hours per week and may be considered as full time/part time/job share
