You will join the large friendly Medicines Management team at Wirral Primary Care Trust working in GP practices to support optimal prescribing
and Medicines Management. This team is well established and has a long
history of successful working relationships with all GP practices, non-medical prescribers and secondary care clinicians. The team is organised into three locality areas, each led by a Prescribing Advisor and a PCT-based team delivering wider Medicines Management functions. Wirral PCT covers a population of 330,000, with 61 GP practices and 86 pharmacies. This post will be based at various locations across the Wirral.

Our Pharmacists and Technicians are integrated into GP practice teams.
They provide cost effective and evidence-based prescribing advice,
implement cost saving strategies, perform medication review and clinical
audit, review prescribing systems, support implementation of NICE Guidance and NSFs and run face-to-face clinics.

A referral system for Health & Social Care professionals to refer patients for medication review to the Medicines Management team has already been established and this is being developed further. We are a forward thinking PCT and have recently been successful in being selected for a Commissioning for Integrated Medicines Management Collaborative, with a focus on improving prescribing and Medicines Management outcomes for falls prevention.

You will be encouraged to develop special interests and to lead specific projects. You may also have the chance to train as an independent prescriber.

Suitable Pharmacists will have relevant experience and a post-graduate
qualification, or the commitment to undertake a relevant course. You will be committed to continuing professional development, and have basic computer literacy and good organisational, interpersonal and communication skills. A driving licence and access to a car is essential.
